The maintenance reports is an easy one - FMCSA regulations require it for all vehicles under a carrier's control.

I thought under 10K did not count....I never had to do it at E-1 nor Dynamex.....this is the first time I've had to...

Maint. Reports are not that big of a deal for me. If when we scan our report and not have it send an error message back to us, that would be great. I don't know if it went through or not. Should have an option for scanning maint reports. DOT however, is a pain. Hate them. Load-1 and their Ins. company could come up with a form we take to our Doctor and have it filled out. But then, they would say "Same as DOT requirements" we are doomed I tell ya. Got mine done yesterday, good for 2 years.

For the maintenance reports, for any vehicle under the carrier's control for 30 days or more, it's "Every motor carrier and intermodal equipment provider must systematically inspect, repair, and maintain, or cause to be systematically inspected, repaired, and maintained, all motor vehicles and intermodal equipment subject to its control."

The annual inspection only applies to "Every commercial motor vehicle," though.

Then there's Company Policy.

policy - n, a definite course of action adopted for the sake of expediency or facility.

I would think by now that the Other Equipment section would be updated from a Nextel with GPS tracking to reflect QC use.

Otherwise, we're all in breach of contract.

Another big reason for company policy on those two items is liability. if they as a company do not make sure you are physically fit and your vehicle is in good repair then the liability shifts alot towards the company in the case of a accident. not that it keeps them non liable but it does show they took the proper steps to be safe.
