A couple of issues here. First, it is important to understand that most cargo vans have GVW of less than 10,000 lbs. and are not subject to DOT regs. Once the van with GVW of less than 10,000 lb. GVW is capable of hauling placardable quantities of hazardous materials (meaning the carrier will use the van to transport placarded hazmat and the driver has a hazmat endorsement)then the van becomes a CMV (Commercial Motor Vehicle) and is subject to the regs.

Trucking companies can adopt their own requirements that exceed the DOT regs. In other words, the trucking company can require all of their van drivers to pass a DOT physical and drug screen, and to complete a DQ (Driver Qualification) file with all of the DOT mandated forms as part of their "company" policy. These company policies are generally in place to protect the interest of the trucking company, the insurance company that insures the trucking company, the owner of the vehicle (van) and the driver in the event of an accident. Most frivolous lawsuits are won by an attorney proving that the driver was not qualified, or the vehicle not properly maintained. Thus the importance of trucking companies requiring more than the minimum.

The answer to your core question is that your friend will need to apply to the various companies to see who will accept him/her if unable to pass a DOT physical due to the diabetes. I have found that most folks that take pills to control diabetes are able to pass a DOT physical, though I am not the physician.

i have diabetes drove for over a year as long as you can keep your sugar levels under control via diet or pills you can continue driving any size vehicle as long as you are not reliant on insulin however you do have to get dot physical yearly instead of every 2 years
