What would cause this?

Probably too much weight. Good for some vanners to watch. Could also be a bad axle, wheel bearing and such. Which usually go bad from poor maintenance or carrying too much weight.

Loose nuts, stripped nuts, wrong nuts, nutty driver or mechanic(forgot to tight the nuts)

Since there's is no drive shaft seen, it's most likely nuts related issues.
