I have gone to the linc that PANTHER offers but can not get it to work.
Ah, the famous "it" word. What doesn't work? The link, or the Transflo Program itself? Here's the Transflo link:

Click the link, and then save the file to you hard drive (in a place that you can find). Once saved, double click the program to install it. Then run through the setup to enter your information, PTWT, and pick you scanner.
I am running win7, can some one help me.

No such program. Well, Adobe Actobat will load them in, if you have them in PDF format.
Here's the ones I put together.
Panther Forms
The Zip contains 4 files, the Quarterly Maintenance Report, ViaSafe Cover Sheet, Transflo Cover Sheet (which you don't really need, all you need is PTWT), and the PARS cover sheet.
With all the forms, you can edit them in Acrobat and save personal customized changes like name, CDL, plate#, etc. and use that as your customized master.
